CAL 101
Granted LOA
Must submit letter of readmission from Leave of Absence (LOA) addressed to
Dr. Vina P. Paz,
College Secretary
The letter must be endorsed by the program adviser and department chairperson.
1. If Leave of Absence (LOA) is due to health reasons, attach a Certification Form from the doctor stating that you are fit to enroll (validated by the UP Health Service or UPHS).
2. If on leave for one year or more (whatever reason), get from the CAL Records Section a College Admission Slip addressed to the UPHS, and present the medical certificate with “Fit to Enroll” remarks and submit this to the CAL Records Section. Get from the CAL Records Section a college admission slip addressed to OUR, and submit this together with the accomplished student directory, 2 x 2 picture, and medical certificate. Submit this to the Admission and Registration Section of the OUR to obtain the University admission slip. Submit the University admission slip to the CAL Records Section, FC Room 1095.
Readmission from AWOL
For those who went on AWOL for one semester:
1. Write a letter of appeal for readmission addressed to the College Secretary.
2. Have the letter endorsed by your Program Adviser and Department Chair.
3. Submit the letter to the Office of the College Secretary (OCS).
4. Wait for the advice of the OCS’ staff. You may follow up the status of your case three working days after
submitting the letter and other required documents (specified by the OCS’ staff).
For those who went on AWOL for more than one semester:
1. Write a letter of appeal for readmission addressed to the Dean.
2. Have the letter endorsed by your Program Adviser and Department Chair.
3. Submit the letter, together with the filled out Readmission Appeal Form and True Copy of Grades, to the OCS for
the College Secretary’s evaluation and endorsement.
4. Wait for the advice of the OCS’ staff. You may follow up the status of your case five working days after
submitting the letter and other required documents (specified by the OCS’ staff).
For those whose readmission appeals are approved:
1. Secure your College Admission Slip from the OCS; and present it to the University Health Service (UHS).
2. If you are found “fit to enroll,” get the UHS-issued Medical Certificate; and proceed to the Cashier’s Office to pay
the AWOL fee amounting to PHP 225.
3. Present the receipt to the OCS; and secure another College Admission Slip.
4. Submit the second College Admission Slip, together with the accomplished Student Directory, one 2 x 2 picture,
UHS-issued Medical Certificate, and the approved letter of appeal for readmission, to the Admission and
Registration Section (ARS) of the Office of the University Registrar (OUR). After submitting the aforesaid
requirements, get your University Admission Slip.
5. Submit the University Admission Slip to the OCS.
Application for Graduation
A student needs to apply for graduation in order to be included in the list of candidates for graduation.
1. Application should be filed a week after registration or at the deadline set by the OUR.
2. Fill out the form at CAL 101.
3. Submit the form, together with an NSO birth certificate at CAL 101.
LATE FILERS:
1. Write to Dr. Maria Vanessa Lusung-Oyzon, University Registrar. The letter should be endorsed by the program adviser, the
department chair, and the college secretary.
2. Once the request is approved, pay the late filing fee at the Cashier’s Office, PNB Building.
3. Fill out an application for graduation form.
4. Get a Student Disciplinary Council (SDC) clearance.
5. Submit the application, receipt, the Registrar-approved letter, and the clearance at the Records Section, Office of the
Secretary.
Note: Make sure that the application for graduation has been accomplished.
MRR Waiver
When the student exceeds the years set by the university to finish a course, she/he will not be eligible to enroll.
For the two-year certificate courses, a student is given three (3) years to finish the program.
For the four-year baccalaureate courses, the limit is six years.
1. Write a letter (secure 2 copies) requesting an extension of the Maximum Residence Rule (MRR) addressed to Dr. Jimmuel C.
Naval, Dean, College of Arts and Letters through channels (endorsed by the program adviser, department chairperson, college
secretary) and get an Appeal for Extension of Residence (Waiver of MRR) form at the CAL Records Section. Attach the
program/plan of study (courses to be taken in the remaining semesters).
2. Submit the letter to CAL 101. Follow up the result after one week.

1. Fill out the request slip form at CAL 101 or access it online.
2. Pay the evaluation fee at the Cashier’s Office, PNB Building. Or follow these online payment options:
